I think our situation is more analagous to climbing. Bolts are removed because they are poorly placed, superfluous or unsightly. Thus filling it over and making it nice is a good thing.
In a cave situation, the reason for removal sounds like more often because of poor rock in that particular location. Which can only be discovered by drilling into it. Thus it makes sense to leave the holes as warnings to others.

As I recall, the ASCA recommends filling and disguising old bolt holes, and the National Speleological Society takes an alternate view, i.e. that some evidence of a previous bolt should be left in place to avoid misleading subsequent bolters about the strength of the nearby rock.
Is this an issue?
